What is USPS Form 1583?

USPS Form 1583 (The Application for Delivery of Mail Through Agent) authorizes a CMRA to accept and handle mail on behalf of your Iowa business. There is no filing fee.

What information is required on Form 1583?

You will need to include the following basic information:

  • Your name and street address
  • Copy of your photo ID & proof of address
  • Your Signature

Note: Although Form 1583 requires you to list your home address and phone number, this information does not go on public record. And, we will never sell your personal information to third parties.

USPS requires two forms of ID: a photo ID and an address ID. Here’s a complete list of accepted IDs. You must submit one ID from each list.

Yes, a non-US resident can hire a CMRA as long as they meet the ID requirements. Non-US addresses are acceptable.
